We are seeking a home share provider who is passionate about providing exceptional care and support to a remarkable woman with a moderate developmental disability and some physical and behavioral care needs. You will share your home with the individual, provide hands-on support for activities of daily living and health care assistance. You will help her to use her walker in your home and a wheelchair when out in the community. Additionally, you will transport her to and from a day program in the community several days a week and to medical appointments. A few weekends during the month she will go to other homes for respite care.
